Our hospital does offer fee-for-service out-patient visits with an IBCLC. Most visits are in the $15-30 range - we try to keep it in the low range unless mom needs real in-depth help. Many visits are for weight checks or advanced hand-holding, although we do see our share of tough cases. This is in addition to telephone calls to all of our first-time breastfeeding moms and any others who were seen in the hospital and/or are having problems within a few days after they go home and for as long afterwards as they desire - some like to be called once a week or so for a while just to touch base until they are settled in with breastfeeding. How do we find time for all of this follow-up? It is very difficult. This is in addition to floor work which is never as thorough as we would like it to be. I mostly teach bf classes and help on the floor when they are really short on help. The students seem to appreciate the fact that they will have access to outpatient visits if they need them.
