I may have posted this awhile back but it bears mentioning in this
discussion. I recently worked with a young woman who had small breasts
with less than protactile nipples and to top it off, a baby with a short
frenulum. This baby had a very difficult time sustaining a latch from
the get-go. She had sore nipples within 48 hours and came back to me for
assistance. When I suggested she mention this to the pediatrician at the
1 week check (baby was now receiving breastmilk from a bottle --since
mom was sold on the superiority of breast milk) she was skeptical but
agreed. When I did my follow up call to inquire about the weight, she
says the Ped "laughed about the idea" and said it was normal. Mom
declined to pursue it any further and declined to attempt to put baby to
breast now or in the near future. Last I heard she was still
pumping...sigh.
