Dear Lu,
The size of the hole will vary from mother to mother. I have talked to moms who have done this and it worked well for them. Pulling down the flaps on many nursing bras can open too large of an area for some moms and provides very little support or help while actually nursing the baby. This way the bra can be individualized for that mom. They can even sew in flaps themselves. I think I may actually have the directions stashed away somewhere - if you want them, I can perhaps dig them out. Also have had moms who had success using the sling set-up shown in "Bestfeeding" by Renfrew, Fisher and Arms. Hope this helps.
