Beyond mechanical obstruction that might be surgically caused....... First,
I agree with you that it is possible that the milk cannot easily flow *out*
through the nipples. I once had a mom whose milk would come in on both
sides, but milk would only come *out* one side, which I concluded to be due
to a malformation of the nipple-- if I recall correctly, the nipple was
unusually formed as well.  Or, mom could be suffering from an impaired milk
ejection reflex;  I've seen a similar phenomena in a mother who had a hx of
sexual abuse; she also "had" to have her labors induced.
