Dear Jamie,

Welcome to Lactnet.  Your mom sounds like an excellent candidate for a nipple shield.  Attach the shield after the mom has pumped a few minutes and drawn the nipple out a little. Turn the shield almost inside-out and position over the nipple, which should then be pulled, at least a little bit, up into the shield.  You can try the regular or the small shield, available from Medela.  Make sure that the baby is up on the areola, even when using the shield.  The shield will help stimulate the baby to suck.  Once baby is comfortable at breast with the shield, can begin weaning from the shield.  Monitor for pees, poops, weight gain.  Would much rather see a baby at the breast with a shield than off the breast getting EHM by whatever means.  Is also much less time-consuming for mom if she can dispense with part or all of the pumping
