I don't know who originally calculated the 24 hr milk intake "formula" but
it is calculated based on baby's wt. The formula you are using is
wt (lbs) x 2.5 ounces = 24 hr intake
so 8 lbs x 2.5 ounces would be 20 ounces per 24 hrs needed.
Realize that this is a ball park figure so that baby's metabolism, activity
level, other metabolic needs can factor in. But it is a good formula for
figuring when supplementation is needed.

Others use a wt (kg) x ccs/kg so that one formula would be
wt(kg) x 150cc/kg/day = 24 hr intake
3.6 kg x 150cc/kg/day = 540cc/day or approx 18 oz. I have seen 180cc/kg/day
or 200cc/kg/day recommended in different scenarios so maybe that is where
the discrepancy comes in. Also some folks use 30cc per ounce and some folks
use 28cc per ounce, so there could be discrepancy there too. If you want a
reference look in a neo or peds text and everyone can agree on that. Maybe
the Harriet Lane handbook which is what alot of neos and peds residents and
staff use. Hope this made sense.
