any other ideas to help prevent what looks, at this point, inevitable

you don't say whether you've had nipple/breast thrush while breastfeeding.
If not, I would just rinse mouth as directed and take acidophilus caps. And
watch out for nipples getting tender / shooting breast pain. If you have had
thrush while feeding, I would suggest taking oral nystatin while you're
taking the antibiotics (2 caps 3x/day) as well.
