Forgive me if this has already been said.  I am catching up on old digests
after a terrible drought period (no mail, horrors) and haven't got up to
date yet.
We have had essentially the same high BF rates for the last 10 years or so--
not much change in initiation rates, anyhow, though duration has increased
as maternity leaves have lengthened.  The SIDS rate fell to half its
previous level and has stayed there ever since, when we stopped insisting
that babies sleep in the prone position.  We did that about two years after
the UK did, in the early 1990's.  Now we are working on reducing the number
of deaths  due to parental smoking.
If head shape is a concern, try the Kurdistani technique: a cloth bound
around the baby's head from birth, to encourage the development of the head
shape thought to be loveliest in their eyes.  Any Kurdistani grandmother can
demonstrate it for you.  Or just start developing an appreciation for a flat
occiput, highly prized in most parts of Eastern Asia.
