I watched a show on the Discover Channel about plastic surgery.  They showed
a woman having breast implants inserted with a a periareolar approach.  As
the tissue was cut, it was cauterized.  Any surgery around the nipple or
areola that involves cauterization would weld shut (it seems to me) the
ducts.  The woman could engorge under the endocrine factors that dominate
early lactation, but couldn't deliver the milk to the surface since the
canal structure would be severed.  Consequently, due to localized feedback
inhibition  (autocrine control) the unremoved milk would trigger weaning.
