Speaking personally/anecdotally for a minute, I found when bf my first two
kids that the sinuses up near the armpit did not drain as well as those lower
down, and so I spontaneously came up with basically exactly what that
midwifery newsletter described to help them drain.  But it was not in concern
for how much the baby was getting -- she was always allowed to stop when she
wanted to come off, after all -- but rather out of concern for avoiding
engorgement up there in the tail of the breast, to which I was prone right up
until weaning if that breaast was very full.

So that may be another way to think about when & whether this "variation" is
appropriate to use.
