I know there have been a lot of responses to this question and I have
nothing new to add, except timing of what everyone has suggested.

With switch feeding to increase a poor supply i usually suggest feed
normally side one, use breast compression with side two, breast compression
side 3 and SNS with side 4.  And often I will suggest not starting the SNS
until after lunch (depending on just how bad the supply is) because most
mothers tend to have a better supply in the mornings.

With this particular case too - which most of us are agreed is gaining
weight within normal parameters (though formula *may* be the essential
ingredient for that - give this Dr the benefit of the doubt) - by not
offering the comp until the very end of the feed the breasts are getting
maximum stimulation (incl. breast compression) and baby may be full before
getting to the formula, which may not have been needed anyway.
