Thanks to  everyone who replied about the situation I posted.  I am overwhelmed with the number of replies I got privately.   Many great suggestions.  I loved the idea of the skin to skin in sling under her clothes (thanks Johanna).
 I spoke to the mother today (it was only yesterday that I saw her) and she managed one skin to skin nap in the morning while other kids are in school, and she increased pumping frequency from 4 or 5 to about 8 times though unable to get the better pump as of now.   She has  decided to take the approach of not offering the breast for a few days while trying to build supply.   Home conditions are not good for co-bathing, but night time skin to skin and the sling under clothes idea both appealed to her.   I'm not sure yet how this will progress but this mother REALLY is trying.   I think motivation is about 90% in these difficult cases.
