Dr. Jack, last year you emialed me the formula for your APNO as follows:

mupirocin 2% ointment (10 grams)
nystatin 100,000 unit/ml ointment (10 grams)
clotrimazole 10% (vaginal cream) (10 grams)
betamethasone 0.1% ointment (10 grams)

that is different than what you recently posted to lacnet.  could you
illuminate on the reasons for the changes?  it seems you have removed the
clotrimazole?  why is that, or is it not across the board and case by case
isntead .. . ?  thanks.
