Asked innocently - not to be devil's advocate:

Isn't it possible that this child recognizes that mom's milk should come from
mom, and not a bottle, and therefore doesn't want it from the "wrong"
container?  My second daughter would never take a bottle, regardless of
nipple type, even if it had my milk in it, and even if it was dad giving it
(actually, I don't think I ever tried myself.  If I was there, there was no
point!!).  Fortunately, I didn't return to work after her birth until after
she was easily taking a cup, so it was never a real problem (it just meant
when my husband and I tried to "go out" we were never gone more than an hour!)

While I agree that the child care facility might be a problem, it seems like
there really could be refusal on the part of the child of "wrong packaging".
Would it work to "wean" the child from formula to EMM, by mixing the 2 in
various amounts until the baby was getting EMM?  Every time WIC changes
formula contracts here, I have a few patients that don't like the new
provided formula, but we've always been able to get them from one to the
other by slowly transitioning them.  (We have an abysmal BF rate here, which
our overworked LC, her advisory committee and several doc's are trying to
change, but it's slooow).

Just some thoughts, and I welcome the observations of others with different
experiences.
