I had parents here the other day with a 2-3 week old.  They were all set to 
make a supplement with powder.  I sent dad out to drugstore to get some 
ready to feed.  I discussed the hazards of powder and told them the whys and 
wherefores.

Ready to feed-expensive and convenient
Concentrate, not as convenient
Powder, convenient and least expensive - Ok when baby over one month.
Explained that first 2 are sterile and powder is not.
