Hi Judy,
It is the mother's decision. The LC can and should provide the
medication info to the mother, and the pediatrician can certainly give
his/her opinion on the safety profile of the med. The mother then
makes the decision to send the milk home to the baby or to have the
baby come and bf, if possible. If the medication is not one of the
safer ones, then the mother can confer with her doctors, pharmacy,
pediatrician, whomever to see if she can change the meds, or change
the timing, or bf around the peak, etc.

 That's as far as I would go with it.
