Mary asks about day 1-3 and use of vitamin D.  I read the ENTIRE American Academy of Pediatrics report on Vitamin D for infants and looked at the cite research. There was simply ZERO evidence about whether starting on day 1-3 or waiting until 2 months (according to the previous recommendations) made any difference because no one has studied the impact from day 1-3 and shown that it improves vitamin D status.  No one has compared starting at day 1-3 with waiting until later.  

Personally, I find that many parents give up on the drops entirely because babies have trouble with them that early. A neonatologist and myself had our comments published in the comments section.  I had to sign a bunch of stuff before they put my comments in.

What needs to be designed is a study of infants who are started at 2 weeks and at 2 months with a comparison of vitamin D status.  I really think it is unethical to start vitamin D so early before even the basics of breastfeeding are established.
