Have you explored issues such as overactive letdown that can cause
aversion to the breast?  Does the baby gulp loudly, seem to struggle to
keep from choking, etc?  Has the mom tried lots of skin to skin contact,
nursing in the bath, latching and nursing while standing and walking?
