I am looking for some formatted information to be handed out ( maybe even
signed) by a mother, inpatient who requests her infant be given formula.
Ideally this info would educate her about the risks of bottles\ nipples in
the immediate newborn period and the consequences of feeding formula. I know
I have seen this tear off sheet\ informational sheet somewhere, but where?
