We are considering billing for our outpatient lactation consultations at our 
hopsital. Our Lactation Services is staffed by myself, a non-RN IBCLC and 2 
CLC's who work under my supervision. We see about 5-7 outpatients weekly.

I am interested in the following information from any of you who are currently 
billing for outpatients:

1) How did you come up with your rates?

2) Do you charge the patient who is then responsible for getting insurance 
reimbursement?

3) What percentage of your chargeable visits are actually covered by 
insurance?

4)  How do you handle Medicaid patients?

5) Does insurance generally only cover if billed under a licensed health 
professional?
