Yes, I use the consent forms -
Though frankly I started using them not because of the moms, for I was
comfortable with the education I was giving them and documenting this.

I started using them when I found them disappearing like hotcakes on days I
wasn't on the floor - used by nurses who weren't taking the time to help attach
babies.

Once I started insisting that a form be completed (in duplicate - one for mom
and one for record) and signed by the mom, their use slowed down.  I would like
to say they should only be given by and LC, but as I am very part-time, this is
not feasible.  If moms aren't given an option while at the hospital, they mostly
quit - few have the persistence to go see the MD/LC who has a clinic nearby.
