One idea would be to culture the lesions on baby. Also culture the nipple.
One possibility would be herpes.
One possiblilty would be staph.
One possibility would be hand, foot, mouth disease (I think that's what it's
called).
It also sounds like oversupply/overactive letdown with baby biting down to
curb the flow, might also be a factor.
One more thing, a tight upper (labial) frenulum.
