I believe Pfizer has one of these little booklets associated with their
Z........ product, but I'm not sure if it was one time promotion or
available on an on-going basis.  Anyway most states provide a free
immunization record.  We just write NJ and get 1000s at a time.  NJs are
bright yellow and index card stock.  I have seen others that parents carry
from US and all over the world.  Personally I prefer these as they are
standardized.  What I really like is the state which  also provides a
little plastic sleeve to keep it in!   There- see I can be nice when I've
