I had one situation with a mom whose breasts LOOKED similar, but when one breast didn’t release any milk, I moved to a large flange - and milk flowed!  This was long before we had all the different sizes of flanges, so I was using the old glass medela flange (HUGE) on a breast that did not look that large - but it worked....
