I think an excellent place to look would be the Baby Friendly Hospital
Initiative (BFHI) info. I don't have in front of me but the step that
addresses no supplements unless medically necessary goes into all the
medical indications and what are truly non-indications. The assessors have
the full criteria and of course there are references for all the steps.
There is a publication or document "the evidence for the 10 steps" or
something like that. I'm sure if you contacted the BFHI people in your
country they could get you the literature.
Also the Academy of Breastfeeding Medicine has great protocols as stated by
someone recenlty.
