The case you described is beyond my expertise, as well, but I had a thought.
Is it possible for this mom to obtain a babyweigh scale?  You can rent them
monthly from people who rent out breast pumps.  It seems like measuring
intake by weight would be valuable information for the MD to manage care, and
for the mother to nurse her baby directly as well.  If she has an accurate
way to measure intake at the breast, she will know just how much, if any, she
needs to supplement.

By the way, Marsha, I loved your letter about the soy milk/formula.  Way to
go!
