Does baby have a weak suck, posterior tongue-tie?  Sounds like milk not being removed quickly enough and backing up into the ducts.  As milk volume increases in breasts with spontaneous let-downs, there is pain from full ducts.  Could try pumping after feedings to completely drain ducts and see if that helps.

Maybe mammary constriction syndrome ?  Edith Kernerman does some research with this.
