One more thing to add to Nikki's fabulous list:

Have baby latch onto the bottle, cross lips with nipple, hold baby in 
sidelying against breast, encourage baby to open wide and lunge for the 
bottle as the parent tilts the nipple tip so it now aligns with baby's 
tongue. This is usually my first step, trying to bottle feed 
"idiomatically" to make all behaviors as similar to bf as possible.
