Dear Lactnet Friends:

I took the examination for the 3rd time this month. Here's some thoughts
about it.

1) It was worth it to take the IBLCE practice exam so that I was familiar
with the computer system.  I had fun this time, too.

I wish I could have taken the Pearson-Vue tutorial, but it wasn't
compatible with a MAC. This is interesting as a fair number of us have MACs.

2) At least one IBCLC has has been publicly reprimanded for diagnosing.
Diagnosing is not in our scope of practice. Yet there were questions on the
exam that clearly required diagnostic skills. Example. A picture of
something clinical was shown; the question was to pick (out of 4 choices)
what this thing was. There was no answer, "refer to an expert in these
things and meanwhile breastfeeding can continue." No, this was clearly a
diagnostic question, as could be found on a physician's examination.

3) We are taught that evidence is important. Yet there was a question about
something that has only negative evidence about it. The evidence shows that
this something undermines breastfeeding or creates clinically undesirable
situations.  the question was about how to implement this something.

Seems like a conflict to me. How are we to practice in accordance with
Scope of Practice when our own certifying examination asks for skills we
are discouraged from using??

Anybody have any ideas?
