While it wouldn't apply to all variations, I wonder if things like flat or inverted nipples as well as surface conditions relate at all to how the breasts are "dressed" (or undressed) in different cultures.  Does our western preference to "packing them in" to tight bras or push them up beyond simple support contribute to any of these variations?  Just pondering.
