In fact, what I would recommend is writing to that physician and
stating "that is a wonderful idea! Would you like to write it up and take
first author with me as lead author? Or shall I write it up as first
author and you take lead author?"

Either way, your name should be on the paper (and you should influence how
it is written, with care to make sure that the emphasis is on protecting
breastfeeding in the beginning, with donor milk used when the horse is out
of the barn). Congratulations on such a lovely outcome!
