The baby is gaining weight well? At least 7-8 oz per week?
Biliary atresia is one reason a baby will remain this jaundiced.
Normally with biliary atresia, the baby's stools are unusally pale, and
you mention the stools are yellow. Have you seen the stools? Normally an
experienced mother will notice the difference...
Breast milk jaundice (hypothetically something in mom's milk reduces
bilirubin clearance in the baby) does occur, usually the bilirubin level
remains elevated for months, but is not usually as high as 15 at 6 weeks
postpartum. I had one little guy in my practice who had a level of 6-7
for 6 months. Generally this occurs in siblings. Strange that this is
baby #3, and none of the sibs have experienced this. That would raise
the index of suspicion for me that there is something wrong with this
baby, and he deserves a careful evaluation.
