Don't know if you've tried this, but if you are suspecting too much
foremilk, it might help.
Try limiting baby to one side for each 3-hour (or so-sometimes other
time frames have been suggested) period.  In other words, assume
baby nurses at noon on the right side.  Then anytime he wants to
nurse before 3 o'clock, he returns to the right side.  Then the
first time he nurses after 3 o'clock, he goes to the left and nurses
there over the next 3 hours etc.
It's somewhat of an extreme approach, but I've seen it help in some
cases where we suspect not enough "hind" milk and the Ped is talking
"reflux".  If mom is getting uncomfortable, she may need to pump the
"off" side a little.  In the situations where I've had a mom do
this, she has been able to return to more "normal" nursing patterns
in a couple weeks.
