I am not a hcp, and I have never worked in a hospital, but it
seems to me the better part of discretion and protecting the nurse
and the mother, that the door remain ajar if there is no one else
in the room when a male nurse is helping the mother to breastfeed or
do anything else.
I don't know what standard protocol is in hospitals, or whether this
is too cumbersome to be practical, but my experience in doctor's offices
is that male doctors have another staff person (usually female) in the
room when they are examining women patients. I imagine the same is true
of female practiotioners working with male patients as well.
