ISEN-ASTC-L is a service of the Association of Science-Technology Centers
Incorporated, a worldwide network of science museums and related institutions.

We had someone donate money to our museum last week for scholarships to our
summer camps and workshops. Our museum currently does not have any thing
set up to for this. Could you please share any forms or wisdom if your
museum has offered these? The donor was not pacific about how we use the
money. We are thinking about offering full or partial scholarship to help
more families out. We are not sure what kind of information we should ask
for (besides the basic; ie: address, phone number, etc). Our museum is
located in a small community. I do not think we will be have many
applications returned but you never know. I like to be prepared and I hate
the duh moments when you just thought of something you should have placed on
the form. Also if you offer partial scholarship, how much does your museum
fund or do you have a sliding scale based on need?

Thanks!
