google up "3m press on prisms"

these are used by optometrists to to temp adjustments for people with "lazy eyes".

they are about 50 mm diam ( 2 inch) and about 2 mm thick. They are freznel prisms. and they are slightly tacky
on the back ( like some of the people I work with), and so can be pressed onto existing glasses, or safety goggles.
they come in strengths up to 20 or 30 diopters, which will be plentry strong enough for anything you'd want to do to the public. they cost about $20-40 a set.

I am looking to buy or make a few sets of prism glasses (the kind that shift
your field of view about 30 degrees to the side).  I am working with a group
of neuroscience graduate students and researchers at Yale to create a
science outreach program to local schools. We were thinking of including an
activity where students toss a ball at a target with the prism glasses on
and then see how they adapted to the glasses once they take them off.   I
know I have seen these demonstrations in a number of science museums so I
imagine someone out there should be able to tell me where I can purchase the
components (and approximate costs) and how to assemble some durable sets of
prism glasses.
