Hi All,
I saw a mom today with her 5 day old. She has attempted brfdg several times
and states it is extremely painful and has not been able to tolerate more than
a few minutes.  Her mature milk is in with slight engorgement, nipples errect,
intact, no signs of damage.  Baby term, healthy, no digital exam: strong suck,
grooves to my finger but can only extend tongue to gum line not over gums.
Attempted baby on Lt br but baby acted like he could not grasp enough breast
tissue to latch, very frustrated.  Showed finger feeding with a syringe, also
showed cup feeding, took in 1 oz then tried on Rt side, when mom expressed sm
amt and pinched  up am amt br tissue baby latched right on nice wide angel,
flanged lips but could not see tongue. Mom stated pain was unbearable but
encouraged her to hang on a few minutes while I assess and observed. Baby did
get a let down with a nice 1:1 suck/swallow. Then had mom release nipple,
slightly elongated with some compression.  Mom has M----- L------ el. pump
which is also uncomfortable but not as bad as the baby.
I have a referral to ENT to clip the frenulum, which they said they will do.
My question is has others had similiar cases? The short frenulum is the only
problem I observed.  Meanwhile parents are getting a supplemental system to
finger feed and mom to pump.
