  In response to South Africa's caesarean rate - Linkwood Clinic is a birth Unit in Johannesburg with a caesarean rate of 16% amongst midwives clients and about 22% of obststerician's clients (mostly breech, placenta preavia etc) We have debated at length with the whole team VBAC's - noone calls them trial of labour - and we dont follow the private obstetrician's trend of repeat caesars for previous caesars. The unit's philisophy is strong. Thus we have a virtually 100% breastfeeding rate. (We could use a full time breastfeeding problem solver though !) We also have the paediatricians and anaethetists buy into skin to skin at birth with a caesar - that really helps. We dont send caesar moms to recovery but bring them straight back to the unit with their babe tucked in on their chests. We keep them on the theatre trolley (really comfy trolleys) for the first 6 hours as we can position the back of the trolley easily (Most of our beds are ordinary double beds) Thus the first priority is to facilitate latching and skin to skin.
