I have had about 3 moms in my years of working with moms who cannot have a
letdown to a pump.  I've had them come in and try every pump I have in my
office with no success unless they have the baby feeding on one side while
pumping the other.

MER is depended on a hormonal response and it's a conditioned response
especially the older the baby. A mother's body becomes conditioned to her
baby's suck and her baby can stimulate the "love" hormone oxytocin much
better than a machine.
