We use wufoo.com for all of our forms. It's 24.95 a month for the lowest
level that includes payment integration utilizing paypal or google checkout.
Their forms can be used for surveys, memberships, field trip reservations,
etc. So, it's not specifically for events, but I think it would work for
that as well. All the information is collected in a database that can
produce various customized reports and spreadsheets.
