I would have to say that in my area of the Deep South, yes, some mothers
would go through the pain of terrible engorgement, risking their own and
their babies health, even after being "educated" about the superiority
of Breastfeeding, and never put the babe to breast.

I have seen moms in clinic wearing binders, popping Darvocet for the
pain, in for their ??? visit to the nutritionist because baby will not
keep any formula down. I have tried to open discussions with these moms,
asking them to please, for their own health take off the binder, and
while their at it, why not offer the baby the breastmilk?
Blank stares, and sometimes foul language have been the replies.
Apparently there is a cultural "thing" about breastfeeding here.
Caucasian women are just too delicate (therefore feminine) and African
American women are too well educated (therfore far removed from any
wet-nurse ancestor) Those of us who do choose to breastfeed are usually
regarded as "Those Down to Earth Hippie Types"

Must admit I've been called worse, but I was only BF for 12 wks, and
look how far I've come! Whose to say what those little WIC formula fed
babies may one day do for Humanity...besides cost us all more taxes for
their health care.
