Years back before the pump companies were making different size flanges we  
often would provide an Ameda kit (slightly larger flanges) to a mother using a  
Medela pump. One day I decided to measure the suction pressure using the (I  
think) Ameda kit on the Medela Classic and the high pressure was way above the 
 recommended levels. I wish I could remember more details but suffice to say 
we  immediately stopped that practice. In fact we switched to using just one  
pump company for in-hospital use because the nurses tended to sometimes  
forget which tubing kit to provide for which pump--no flames intended, nurses  have 
lots to remember.  It's great that now that both  major companies have a 
variety of pump flange sizes.
The mom in question with the bleeding nipples certainly could have been  
using too much suction if she were using the wrong pump kit.
