We had a mom give birth, stayed in couplet care, went home at 48 hours. She took babe in for a Day 3 clinic check & he was admitted for high bilirubin. Babe was d/c'd today on Day 5, bili stable and at original birth weight. Someone set up this mom with our only Medela Classic but with our Ameda kit plugged into the air hose where the Medela filter should attach. (Yes, I have a message on the pump about using the correct kit.)
Today, when they first called me for a consult for "painful nipples", I found bloody milk in the bottles and mom really engorged. Thankfully, babe was willing and eager to latch and feed well. Explained in detail how I adjusted the position to make it comfortable, etc. She went home feeling better about BFing.
Question: Did the wrong kit on wrong pump cause her bloody nipples? I know the physics are off. The compartment size is different between the two kits and swapping will make the vacuum too strong one way and too weak the other way. Can someone give me the details on this so I can explain to the staff good reasons why this is NOT to be done ever again?
