All these factors - weight loss, firm engorged breasts, painfree
nipples, small vol of milk transferred to baby - are on the normal
spectrum on day 4. Not being able to get much milk out with a pump is
also on the normal spectrum, and does not necessarily reflect what
goes on with the baby. Hand expression is sometimes gentler and more
effective with engorged breasts, too.

I'd want to know about the behaviour of the baby on the breast...if
the baby is miserable or fighting, for example, there may be an
issue....and yes, the medical history of the mother may bear some
thought.

But why was the mother referred?
