First I would like to say that I do not believe that an LC can do hourly rounding. I round on all breastfeeding mom's first off to find out when the next feed is and set my day accordingly. Hourly rounding cannot occur if you have 8-10 couplets that you must witness a entire feed, LATCH score it, education, etc. Our protocol is that we round every morning quickly to assess feed times and evaluate the feedings overnight. We then start attending feeds. We witness one entire feed per patient per day. While with the patient we do our education. I would like to hear from others but this seems to be the profile of most LC's days that I have talked to and that were part of the discussion at ILCA last year.We somehow fit in our outpatients in between our inpatients. It can be frustrating. Hourly rounding should be the responsibility of the primary care nurse.
