Has the baby had genetic testing? I've had several mothers going 
absolutely crazy trying to get more milk into their baby, and baby 
wasn't having it. One baby turned out to have a rare microdeletion 
syndrome (Phelan-McDermid syndrome), another had extreme reflux and 
needed tube feeding, another has not yet been diagnosed but an entire 
feeding team agrees that there's something wrong with her feeding skills 
(she's also getting extra breastmilk through a tube).
It's not always the mom (but unfortunately, sometimes it is).
