The mother with persistent thrush (assuming the Diflucan Rx has been
intensive enough) might have an immune deficiency.  After all, she has lost a
lot of weight, has persistent thrush, generally feels poorly, and has skin
rashes.  Dermatology would help to clarify the rashes; immunology might be
necessary to rule out immune disorder,  "Immune disorder" could be any one of
a large number of conditions including hidden carcinomas, autoimmune
disorders, and AIDS, among many, many others.
