Because babe was jaundiced when admitted and not feeding well (defined as 30 min q 3 h) at that time. I was called in a couple of hours before discharge, after the bili lights had been discontinued, on Day 5 of life. The MD said she told mom she could feed babe whenever babe wanted, then sleep under the lights, but the RNs, who are there 24 hours, always press for finishing up the feed in a business-like way - which leads to bottle feeding formula or mom's EBM. She was pumping ineffectively - not often or long enough to avoid engorgement. Mom said the pump hurt and feeding hurt. So when I saw her, babe could still latch and drink, and soften one breast while the other amost visibly grew larger and leaked copiously. I helped her improve the latch and she reported it was more comfortable, much less pain, and she began to relax as babe fed during my consult.

My first question on this is - why pump?  Especially, why pump this early,
when the baby is "willing and eager to latch and feed well"?
