There are two great articles that go into this in great detail. Mothering
Magazine's July/August 2002 and La LEche League's New Beginnings Sept-Oct
2002. Here's what's in the Mom's paper:

What preventative approaches have we taken?
1.)     Natural fluoride toothpaste twice daily and swallowed as child has not
learned to spit.
2.)     Homeopathic medicines
3.)     Herbal mineral supplements
4.)     Tooth brushing with motorized toothbrush
5.)     Calcium rich food eaten regularly including: almond butter, beans,
broccoli, smoked fish with bones, and goat/sheep milk products, (when they
don’t cause hives).
6.)     Juice counts as a sweet an she drinks it probably once a month
7.)     child has her own toothbrush and it is not shared.
8.)     Delayed introduction of solids until about 6 months.
9.)     fluoride coating on teeth every 6 weeks
10.)    fillings as soon as possible when caries appear
