Just talked with a brand new mom-4th child - never developed enough milk
with the first 3 (yes, seems to be real, not just perceived insufficient
supply) - really wants to "succeed" this time - tried SNS with last one
but gave it up feeling it was too cumbersome.
Right breast looks fairly normal except that nipple is closer to the
bottom than the top of her breast-somewhat pendulous. Left breast is
classic "tubular" configuration. She states that that side always
produced less than half what the right one did.
I discussed with her a little about Lactogenesis II and III and that she
needs to keep going for several weeks before making any decision about
whether or not it will "work" this time. I mentioned the Lact-Aid as an
alternative to SNS if supplementation is needed. We plan to do frequent
follow-up and weight checks in these first weeks.
My question to all of you - would starting her on Domperidone right away
to improve her likelihood of producing more milk? It would be no
problem getting an order for it. (Fortunately, her OB is the most BF
supportive one in the area-member of ABM, wife is also one of our LCs.
We also have a pharmacy that will compound it with a couple days notice
to get the ingredients.)
